Patient

Losartan blocks AT1, a switch that a hormone uses to squeeze blood vessels. Your body also makes an active form, EXP3174 (E-3174), which helps block it. Vessels relax, and kidney salt signals change. Blood pressure starts to fall within a week; the full effect can take 3–6 weeks. Blood tests check kidney function and potassium, a salt in the blood. The boxed warning says it can harm or kill an unborn baby. The label also rules out use after allergy to its contents, or with aliskiren in people with diabetes. Ask your care team about these risks.

Limits: Educational draft; individual treatment and final clinical review require a qualified professional.

Questions for this perspective

2 drafted questions in this packet. Targeted additions or replacements for the current guide.

patient-05 — How will I know whether it is helping?

Established. A series of blood-pressure readings shows more than how you feel after one pill. Losartan starts to lower pressure within a week, with the full effect often reached in 3–6 weeks. Keep the record your care team has asked for and bring it to the next visit. For some people with diabetes and kidney disease, the team also tracks urine protein and kidney health over time. Ask which treatment goal applies to you. One high reading is not a reason to change the dose yourself.

Sources: LABEL

patient-18 — Will I need tests or monitoring?

Established. Blood tests check potassium, a salt in the blood, and creatinine, a sign of kidney function. These checks look for risks that you may not feel. For people with chronic kidney disease, KDIGO guidance calls for checks within 2–4 weeks after starting or raising a dose, with timing based on risk. Your care team sets your schedule. Report poor fluid intake, vomiting, diarrhea or new pain pills such as ibuprofen. These can change kidney risk even when the blood-pressure readings look good.

Sources: LABEL; KDIGO

Caregiver

Losartan and its active form, EXP3174 (E-3174), block AT1 switches in blood vessels, kidneys and the adrenal glands. This lowers blood pressure over weeks. Keep a record of readings, falls, poor intake and new medicines. Ask the pharmacist about pain pills such as ibuprofen and salt substitutes that contain potassium. Cozaar contains losartan; Hyzaar adds a water pill. Check the full name to avoid mix-ups. The boxed warning says losartan can harm or kill an unborn baby. Ask the care team for an illness plan if the person cannot keep fluids down.

Limits: Educational draft; individual treatment and final clinical review require a qualified professional.

Questions for this perspective

2 drafted questions in this packet. Targeted additions or replacements for the current guide.

caregiver-11 — How can I help with an inhaler or other device?

Established. Losartan tablets are swallowed; they do not use an inhaler or injection pen. A prescribed liquid does need a medicine measure. Ask the pharmacist to show the dose on an oral syringe and check the strength in milligrams per milliliter. A kitchen spoon is not a safe measure. The label describes one compounded liquid that needs shaking and cold storage. Check the instructions on the actual bottle, because a different liquid may have different rules. Ask the person to show the steps back to you.

Sources: LABEL; PATIENT

recalls — What do past recalls mean for a refill today?

Established. Past losartan recalls involved specific products and lots with unwanted chemicals called nitrosamines. They did not mean that every losartan tablet was recalled. Keep the bottle so the pharmacist can check its maker and lot number against current FDA notices. Ask how to get a replacement if that exact supply is affected. A past recall does not show whether a new bottle is affected today. Report a refill problem early so the care team can help keep the treatment plan on track.

Pharmacist

Losartan and EXP3174 (E-3174) block vascular, adrenal and renal AT1 signaling to lower pressure. About 14% of an oral dose forms this active metabolite,10–40 times more potent by weight, with 6–9-hour half-life. Human interaction data primarily implicate CYP2C9: fluconazole lowers metabolite exposure and rifampin lowers both exposures. Reconcile potassium, NSAIDs, lithium and dual RAS blockade; URAT1-linked uricosuria is a selection consideration, not gout treatment. The boxed warning concerns fetal toxicity; hypersensitivity and aliskiren coadministration in diabetes are contraindications. Check volume depletion, hepatic impairment and formulation-specific suspension storage against the label.

Limits: Educational draft; individual treatment and final clinical review require a qualified professional.

Questions for this perspective

5 drafted questions in this packet. Targeted additions or replacements for the current guide.

pharmacist-04 — What adjustments apply in renal or hepatic impairment?

Established. Cozaar specifies a 25 mg starting dose for mild-to-moderate hepatic impairment; severe hepatic impairment has not been studied. Mild-to-moderate renal impairment increases parent and metabolite exposure, but renal impairment alone does not automatically dictate a lower initial dose. Volume depletion changes that assessment and has a 25 mg start in the label. Neither parent nor metabolite is removed by hemodialysis. Reconcile indication, pressure, volume status, potassium and creatinine instead of using estimated filtration as a stand-alone dosing rule. Pediatric renal restrictions are separate.

Sources: LABEL

pharmacist-06 — Which drug interactions are important?

Established. Fluconazole reduces active-metabolite AUC by about 40% while increasing parent exposure; rifampin reduces both parent and metabolite AUC. Human interaction findings primarily support CYP2C9-mediated activation, despite in-vitro CYP 3 A 4 involvement. Pharmacodynamic risks often matter more: potassium-raising agents can increase hyperkalemia; NSAIDs can impair renal function and reduce antihypertensive effect, especially with volume depletion or diuretics. Lithium concentrations can rise. Dual RAS blockade increases hypotension, renal and potassium risks; aliskiren with losartan is contraindicated in diabetes. The label does not prescribe a universal compensatory dose change for CYP interactions.

Sources: LABEL

pharmacist-09 — What baseline and follow-up monitoring is needed?

Established. Establish pressure, creatinine/eGFR, potassium, volume status, pregnancy plans and interacting medicines. KDIGO 2024 recommends BP, creatinine and potassium checks within 2–4 weeks of starting or increasing a RAS inhibitor, adjusted for current GFR and potassium. Its continuation practice point uses a creatinine rise greater than 30% within four weeks as an evaluation threshold, not permission to ignore symptoms or acute injury below that percentage. Investigate volume loss, NSAIDs and other causes. Arrange a clear escalation and follow-up plan rather than an unqualified acceptable-rise rule.

Sources: LABEL; KDIGO

pharmacist-17 — What preparation, storage and stability rules apply?

Established. Cozaar§2.5 specifies a 200 mL,2.5 mg/mL suspension prepared from ten 50 mg tablets,10 mL purified water and 190 mL of a 50:50 Ora-Plus/Ora-Sweet SF mixture in an amber PET bottle. It prescribes staged shaking and a one-hour standing interval; use the full procedure rather than this ingredient summary. The specified suspension is refrigerated at 2–8°C for up to four weeks, shaken before each use and returned promptly to refrigeration. These conditions do not validate another vehicle, container, concentration or feeding-tube method. Confirm pharmacy preparation and the dispensed beyond-use instructions.

urate — Does urate lowering distinguish losartan?

Established. Losartan inhibits URAT1-linked renal urate reabsorption and has small human studies showing uricosuria and lower serum urate. In a 13-person crossover study of hypertension with hyperuricemia and gout, losartan lowered urate whereas irbesartan did not. This can inform selection among otherwise suitable antihypertensive options, but it is not evidence that losartan replaces established gout treatment. Do not claim every other ARB lacks URAT1 activity: telmisartan inhibited the transporter in an experimental system. Keep transport assays, serum-urate outcomes and gout-flare outcomes distinct.

Clinician

Losartan and EXP3174 (E-3174) antagonize vascular, adrenal and renal AT1 signaling. In RENAAL, the renal/death composite occurred in 327/751 versus 359/762 over mean 3.4 years; mortality alone was not reduced. LIFE primary events were 508/4605 versus 588/4588 over 4.8 years; stroke counts 232 versus 309 have the label’s Black-patient limitation. ELITE II (1578 versus 1574) did not establish mortality superiority over captopril;2022 HF guidance conditions ARB use on ACE intolerance and ARNI infeasibility. KDIGO 2024 guides albuminuric CKD treatment and monitoring. URAT1-related urate lowering informs comorbidity discussion, not gout substitution. Boxed fetal toxicity, hypersensitivity and aliskiren with diabetes require explicit screening.

Limits: Full trial denominators, follow-up, subgroup wording and guideline conditions appear in05; no patient-specific treatment recommendation.

Questions for this perspective

5 drafted questions in this packet. Targeted additions or replacements for the current guide.

clinician-01 — Where does this drug fit in treatment?

Established. The 2025 US and 2024 ESC hypertension guidelines include ARBs among first-line drug classes. KDIGO 2024 recommends ACEi or ARB treatment for defined albuminuric CKD groups, using the highest approved tolerated dose with monitoring. For chronic HFrEF,2022 AHA/ACC/HFSA guidance recommends an ARB when ACE inhibition is not tolerated because of cough or angioedema and ARNI is not feasible; ARNI has a preferred role in eligible symptomatic HFrEF. These recommendations do not add a US Cozaar heart-failure indication. Match guideline context to the selected product label and population.

clinician-04 — How large is the absolute clinical benefit?

Established. RENAAL enrolled 1513 participants with type 2 diabetes and nephropathy. Over mean 3.4 years, the primary composite occurred in 327/751 on losartan versus 359/762 on placebo-based care: a crude absolute difference of 3.571 percentage points. Its reciprocal is 28.004, approximately 28; strict upward integer rounding yields 29. This descriptive count-based calculation differs from the reported time-to-event 16% relative reduction and does not imply a mortality benefit. In LIFE, stroke occurred in 232/4605 versus 309/4588 over mean 4.8 years, a 1.697-point crude difference, with the label’s population and subgroup limits.

Sources: RENAAL; LIFE; CANADA; LABEL

clinician-06 — How applicable is LIFE to another population?

Established. LIFE studied 9193 adults aged 55–80 with hypertension and ECG-defined left-ventricular hypertrophy, comparing losartan-based with atenolol-based regimens, commonly with hydrochlorothiazide. It did not compare all contemporary first-line choices. Final blood pressures were similar but not equal. The label states that evidence indicates the stroke-risk benefit does not apply to Black patients: the primary composite was 46/270 versus 29/263 in that subgroup. This finding limits the supported outcome claim; it is not a race-based contraindication or proof about every losartan indication.

Sources: LABEL; LIFE; CANADA

clinician-03 — What did ELITE and ELITE II establish?

Established. ELITE enrolled 722 older patients with HFrEF and found the same 10.5% primary persistent-creatinine-rise rate in both groups. Its unexpected mortality signal was exploratory. ELITE II then randomized 1578 participants to losartan and 1574 to captopril; median follow-up was 555 days. Average annual mortality was 11.7% versus 10.4%, hazard ratio 1.13 with 95.7%CI 0.95–1.35, without significant superiority. Losartan had fewer adverse-effect withdrawals. Do not convert annual mortality rates into cumulative NNT or interpret this superiority trial as proof of equivalence. Current HF placement requires the guideline as well as historical trials.

Sources: ELITE; ELITE2; HF

clinician-13 — How do comorbidities affect the choice?

Established. Albuminuric diabetic CKD is a strong evidence context for RAS blockade, with Cozaar’s labeled nephropathy population narrower than every patient with diabetes. Hyperuricemia may favor discussion of losartan’s urate-lowering action when blood-pressure options are otherwise suitable; the small comparative evidence is biochemical, not proof of fewer gout attacks. Pregnancy carries boxed fetal toxicity. Review potassium, renal perfusion, volume depletion and aliskiren in diabetes before selection. A urate advantage does not override these risks, and a creatinine change must be interpreted with the full clinical course.

Nurse

Losartan and EXP3174 (E-3174) block AT1 responses in vessels, adrenal glands and kidneys. Lower aldosterone can raise potassium; altered kidney filtration pressure can raise creatinine. Pressure reduction starts within a week and develops over 3–6 weeks. Check ordered blood-pressure parameters, fluid status, kidney results and potassium, particularly after illness or treatment changes. The boxed warning identifies fetal injury or death; flag pregnancy promptly. Hypersensitivity and aliskiren use in diabetes are contraindications. Teach-back should confirm why blood tests matter and whom to call if vomiting prevents fluid intake.

Limits: Educational draft; individual treatment and final clinical review require a qualified professional.

Questions for this perspective

2 drafted questions in this packet. Targeted additions or replacements for the current guide.

nurse-07 — Are dilution, infusion or compatibility checks relevant?

Established. Cozaar is an oral tablet product, with a specific compounded oral suspension described in its label. Intravenous dilution, infusion-rate and line-compatibility instructions do not apply to this product. Oral liquid preparation and administration still require concentration, measuring-device, shaking and storage checks. Do not interpret not-applicable infusion checks as permission to improvise a liquid or administer it through a tube. Ask pharmacy to confirm the actual formulation and route, and use the prescribed administration record and local procedure for any unresolved discrepancy.

Sources: LABEL

nurse-18 — What should teach-back check?

Established. Ask the patient to explain what blood-pressure readings show and why potassium and kidney blood tests are needed even without symptoms. Confirm the agreed contact plan for vomiting, diarrhea, poor intake, faintness or reduced urine, and identify OTC NSAID or potassium-salt use. The boxed warning requires urgent pregnancy recognition and prescriber contact; the label calls for discontinuation as soon as pregnancy is detected. Hypersensitivity and aliskiren use in diabetes must be flagged. Use the actual prescription and local escalation process rather than inventing universal hold thresholds.

Student

Losartan and EXP3174 (E-3174) antagonize AT1 receptors in vascular smooth muscle, adrenal tissue and the kidney. Blocking Gq/11–PLC–IP3/Ca2+ signaling reduces contraction; less aldosterone changes sodium and potassium handling, while efferent arteriolar effects alter filtration pressure. Blood pressure falls over weeks. Renin and angiotensin II rise through feedback even during effective blockade; AT2 remains available. Parent losartan is active, whereas the metabolite has greater functional potency and a 6–9-hour half-life. Reversible insurmountable antagonism in an experiment does not mean covalent binding or prove clinical superiority over another ARB.

Limits: Educational draft; individual treatment and final clinical review require a qualified professional.

Questions for this perspective

5 drafted questions in this packet. Targeted additions or replacements for the current guide.

student-03 — How does AT1 blockade lower pressure?

Established. Angiotensin II activates AT1 receptors on vascular smooth muscle, including Gq/11–PLC signaling that generates IP3 and mobilizes calcium for contraction. Losartan and EXP3174 (E-3174) inhibit that receptor response. Adrenal aldosterone signaling and kidney hemodynamics also change, connecting receptor action to sodium handling, potassium risk and filtration pressure. This is a selected pathway diagram, not every AT1 signal: the cited smooth-muscle study distinguished early PLCβ and later PLCγ responses. Mechanistic plausibility explains what to measure; clinical outcome trials establish whether a treatment strategy helps a defined population.

Sources: LABEL; PLC

student-05 — Which PK facts explain the time course?

Established. Losartan has about 33% oral bioavailability and reaches peak concentrations around one hour; its active metabolite peaks at 3–4 hours. Parent and metabolite half-lives are approximately 2 hours and 6–9 hours, respectively. About 14% of the oral dose becomes the active metabolite, whose exposure is about four times parent exposure. These values help explain why parent half-life alone cannot predict treatment duration. The label reports a substantial blood-pressure effect within a week and maximal effect over 3–6 weeks. Plasma peaks, receptor effects and chronic clinical response are different measurements.

Sources: LABEL

student-08 — Is losartan an inactive prodrug?

Established. No. Losartan itself competitively and reversibly antagonizes AT1 receptors. Metabolism also produces EXP3174 (E-3174), a more potent active carboxylic acid metabolite with longer half-life. The label describes reversible noncompetitive functional behavior for the metabolite; that is not evidence of covalent binding. Candesartan cilexetil and olmesartan medoxomil provide different examples: ester prodrugs converted to active ARBs. Distinguish a drug that is active before metabolism from one designed for conversion, and do not treat greater receptor potency as an equivalent multiplier of clinical benefit.

Sources: LABEL; METAB; COMPARATORS

student-10 — Which adverse effects follow from the mechanism?

Established. Less aldosterone can reduce renal potassium excretion, raising serum potassium. Reduced angiotensin-dependent filtration pressure can worsen kidney function in susceptible settings, especially volume depletion or renal artery disease. Less vasoconstriction can cause symptomatic hypotension. Fetal renin–angiotensin signaling is also affected, explaining the boxed warning. Losartan does not directly inhibit ACE-mediated bradykinin degradation, so ACE-inhibitor cough is less characteristic, but cough and angioedema are not impossible. The potassium counterion contributes little potassium per tablet; the major hyperkalemia mechanism is hormonal and renal physiology.

Sources: LABEL; ELITE2

student-15 — Why can renin rise while treatment works?

Established. AT1 blockade removes part of the feedback that normally restrains renin release. The label reports a two-to-threefold rise in plasma renin activity with resulting higher angiotensin II levels during treatment. Higher upstream hormone levels therefore do not alone demonstrate failure of receptor blockade. AT2 receptors remain available, but attributing a particular clinical outcome to unopposed AT2 signaling requires evidence beyond this feedback observation. Compare losartan with lisinopril at the pathway level: receptor antagonism and ACE inhibition interrupt different steps and have different bradykinin consequences.

Scientist

Losartan and EXP3174 (E-3174) antagonize AT1 signaling in vascular, adrenal and renal tissues, lowering blood pressure. Established evidence includes label pharmacology, LIFE/RENAAL outcomes and the engineered losartan-bound 9ZXC structure in Skiba 2026. URAT1 inhibition and small human studies support a separate uricosuric action. Probable contribution of persistent metabolite activity to duration is distinct from proving that one functional antagonism assay explains the clinical effect. Uncertain clinical contribution of unopposed AT2 signaling remains a testable question. The 3.4Å entry uses a nanobody-stabilized construct, not an unmodified receptor in a living kidney.

Limits: Model resolution, construct engineering and functional assay conditions limit native-receptor inference.

Questions for this perspective

2 drafted questions in this packet. Targeted additions or replacements for the current guide.

structure — What does the losartan-bound structure establish?

Established. PDB 9ZXC places losartan, ligand LSN, in an engineered AT1 receptor complex stabilized with nanobody AT118-R. The entry reports 3.4Å cryo-EM resolution; the 2026 paper describes 3.3Å for the losartan complex. The paper’s main focus is a nonpeptide agonist, with losartan providing antagonist comparison. This is not an unmodified native receptor in a living tissue. Prior 2024 work already included a losartan-bound AT118-L complex. A static structure supports binding-site interpretation but cannot establish clinical residence time, whole-body occupancy or outcome superiority.

Sources: STRUCT; PRIORSTRUCT

urat — How would the URAT1 mechanism be tested separately?

Established. URAT1 transport assays and small human urate studies support an action distinct from AT1-mediated pressure reduction. Iwanaga’s oocyte experiments show substrate-side and preloading conditions matter, and telmisartan also inhibits URAT1 in vitro. Hamada’s study linked losartan response to functional URAT1, including patients with loss-of-function variants. A proposed follow-up should measure unbound parent and metabolite exposure, urate transport and AT1 signaling separately, with transporter-negative controls and rescue. This is a research design proposal, not a validated assay protocol or evidence that all serum-urate change is transporter-specific.

Sources: URAT; URATECLIN

Chemist

Losartan potassium combines an acidic biphenyl tetrazole with a 2-butyl-4-chloro-imidazole bearing a 5-hydroxymethyl group. Oxidation yields the carboxylic acid EXP3174 (E-3174), alongside an already active parent. Both antagonize AT1 in vessels, adrenal glands and kidneys, lowering pressure. Duncia/Carini SAR supports the ortho acidic group and lipophilic substituents; current human interaction data primarily implicate CYP2C9 in metabolite formation. Skiba 2026 and PDB 9ZXC place ligand LSN in an engineered receptor complex at 3.4Å. The potassium counterion describes the salt; elevated serum potassium follows chiefly from altered renal hormonal physiology.

Limits: Coordinate distances alone do not establish hydrogen bonds, physiological protonation or binding free energy.

Sources: LABEL; SAR; STRUCT

Questions for this perspective

2 drafted questions in this packet. Targeted additions or replacements for the current guide.

chemist-04 — What did the original SAR establish?

Established. Duncia’s nonpeptide antagonist work identified the value of an additional aromatic ring, an appropriately placed acidic group, a lipophilic imidazole substituent and a hydrogen-bonding group. Carini’s series supported an ortho acidic group and tetrazole as an effective isostere, leading to DuP 753. Losartan’s 5-hydroxymethyl group is oxidized to the active carboxylic acid metabolite. These are measured series-level observations, not proof that each substitution independently improves clinical outcomes. Interpret them with exposure, selectivity and the engineered 9ZXC binding model rather than drawing a clinical potency ladder from structure alone.

Sources: SAR; LABEL; STRUCT

chemist-10 — Which transporter claim is supported?

Established. URAT1 is a supported pharmacodynamic off-target for parent losartan’s uricosuric action. This does not mean URAT1 controls systemic losartan clearance or explains all its drug interactions. Transport studies distinguish parent from EXP3174 (E-3174), and experimental inhibition depends on concentration and membrane-side conditions. Human urate data strengthen the biological link but do not supply a complete disposition model. Separate a drug acting on a transporter from a drug being transported by it; renal excretion percentages alone establish neither substrate identity nor the quantitative contribution to clearance.

Investor / Diligence Analyst

Losartan and EXP3174 (E-3174) block vascular, adrenal and renal AT1 signaling, supporting a mature blood-pressure franchise. LIFE primary events were 508/4605 versus 588/4588 over 4.8 years; RENAAL 327/751 versus 359/762 over 3.4 years. ELITE II (1578 versus 1574) did not prove mortality superiority. ClinCalc estimates 59.0 million US prescriptions,15.4 million patients and rank 7 in 2024. Teva announced generic approval in 2010. Historical nitrosamine recalls make manufacturer and lot controls material diligence questions. Valsartan, olmesartan and telmisartan compete within the class; sacubitril/valsartan occupies a different HF evidence position. Next checks are current Orange Book entries, payer data and supply records, not an assumed price or exclusivity premium.

Limits: Utilization is a survey estimate, not revenue. [VERIFY: market, generic-entry and current supply/IP facts before publication.]

Questions for this perspective

7 drafted questions in this packet. Targeted additions or replacements for the current guide.

analyst-03 — How large is the reachable patient market?

Established. ClinCalc estimates 59,030,486 US losartan prescriptions and 15,388,926 patients in 2024, ranking it seventh by prescription volume. These MEPS-derived estimates describe historical utilization, not unique worldwide patients, eligible untreated demand or company revenue. A reachable market model needs a defined product and indication, geography, payer coverage, net price, switching friction and overlap with other treatments. Use the published counts as a demand signal, then build an explicitly sourced funnel. Do not multiply prescriptions by a retail listing and call the result a defensible market size.

Sources: VOLUME; LABEL

analyst-06 — How strong is the clinical outcome evidence?

Established. LIFE recorded primary events in 508/4605 on losartan-based versus 588/4588 on atenolol-based treatment over mean 4.8 years; stroke counts were 232 versus 309. RENAAL recorded the renal/death composite in 327/751 versus 359/762 over mean 3.4 years, without an all-cause mortality reduction. ELITE II randomized 1578 versus 1574 and did not establish mortality superiority over captopril. These are credible randomized outcome programs with different populations, comparators and endpoints. They validate specific claims, not a blanket efficacy premium across every cardiovascular indication. The detailed trial cards preserve adjusted effects, absolute differences and label limitations.

Sources: LIFE; RENAAL; ELITE2; CANADA; LABEL

analyst-12 — What are the next decisive development milestones?

Established. For established generic losartan, a sensible diligence milestone is confirmation of the exact proposed product, current approval holder, formulation and commercial route. A new indication or formulation needs its own target product profile, evidence gap and regulatory discussion; historical LIFE or RENAAL results do not automatically support it. Next evidence should connect the claimed differentiation to a prespecified clinical or formulation endpoint and a feasible manufacturing plan. No sponsor or new program has been selected here, so a calendar launch forecast would be invented.

Sources: LABEL; GENERIC

analyst-15 — What capital and time would a new program require?

Established. A budget depends on whether the proposal is generic supply, a new formulation or a new indication. Losartan’s established approval history does not remove product-specific development, quality, regulatory or commercial work. Request a protocol and sample-size rationale, manufacturing scope, stability plan, jurisdiction and vendor quotes before attaching dollars or dates. Stage the estimate around explicit deliverables and contingency assumptions. The audit supplies no selected sponsor program or costed execution plan, so this page should show the required budget inputs rather than a fictional financing target.

Sources: LABEL; GENERIC; RECALL

analyst-16 — What patents and exclusivity need checking?

Established. Teva’s 2010 announcement establishes a historical generic approval milestone for Cozaar and Hyzaar equivalents. It does not establish freedom to operate for a new formulation, process, combination or indication today. Review current Orange Book listings for the precise product and application, then conduct a jurisdiction-specific patent and claim search with qualified counsel. Distinguish regulatory exclusivity, listed patents and broader process or formulation rights. Generic availability is useful commercial context, but it is not a legal opinion or proof that every proposed development path is unrestricted.

Sources: GENERIC; LABEL

analyst-18 — What would support reimbursement and pricing?

Established. For a mature generic, current exact-product acquisition costs, payer formularies, reimbursement rules and supply reliability matter more than a historical branded list price. The prior site snapshot expired on October 16 and should not support a current numerical claim. A new product would need evidence of incremental benefit or practical value against available alternatives, together with a defined payer population and price assumption. Record strength, quantity, manufacturer, date and included fees for any future quote. Utilization estimates cannot substitute for net-price or reimbursement evidence.

Sources: VOLUME; LABEL

analyst-19 — What does recall history imply for manufacture?

Established. FDA’s 2018–2019 ARB updates include affected losartan products and lots with nitrosamine impurities. This makes process understanding, impurity testing, supplier qualification, change control and lot traceability relevant diligence subjects. It does not show that every current product is unsafe or that every current lot has been independently verified by this review. Ask for current manufacturer-specific quality evidence and recall checks, then assess continuity of supply and remediation history. Historical recall volume alone does not determine manufacturing cost, present compliance or the safety of an unidentified bottle.

General

Losartan blocks AT1, the receptor that receives a vessel-tightening message from angiotensin II. Its active metabolite, EXP3174 (E-3174), helps block the same signal in blood vessels, adrenal glands and kidneys. Blood pressure falls over several weeks. The LIFE trial compared two treatment plans in 9,193 people with high pressure and a thickened heart; strokes were less frequent with the losartan plan. Small studies also found lower blood urate, a separate effect linked to kidney transport. A 2026 structure shows the drug bound to an engineered receptor, building on earlier structural research.

Questions for this perspective

1 drafted questions in this packet. Targeted additions or replacements for the current guide.

other-10 — How was losartan discovered?

Established. Losartan came from a search for small molecules that could block the angiotensin II receptor. Papers from 1990 and 1991 described changes to ring and acid groups that improved receptor binding. The 1991 paper included DuP 753, later named losartan. It was already in development by then. Researchers also found an active product of losartan metabolism, EXP3174 (E-3174). The US label lists 1995 as the first approval year. Later trials tested stroke and kidney outcomes in selected patients. Newer studies show losartan bound to an engineered receptor, adding a molecular view to this earlier work.

Pharmacokinetics at a glance

All twelve rows: LABEL §12.3; onset §14.1.

Parameter Label finding Interpretation
Absorption / bioavailability About 33% oral systemic bioavailability after first-pass metabolism. Food lowers peak concentration and causes a minor, approximately 10% AUC reduction. Do not confuse absorption fraction with systemic availability.
Time to peak Parent about 1 hour; EXP3174 (E-3174) about 3–4 hours. Plasma peak is not time to maximum blood-pressure benefit.
Half-life Parent about 2 hours; active metabolite 6–9 hours. The parent half-life alone does not determine the dosing interval.
Protein binding Free fractions 1.3% parent and 0.2% metabolite, equivalent to 98.7% and 99.8% bound. Interpret in-vitro concentrations using unbound exposure.
Distribution Volumes 34 L parent and 12 L active metabolite. These are apparent distribution parameters, not organ concentrations.
Metabolic pathway Oxidation of the 5-hydroxymethyl group forms active carboxylic acid; in-vitro CYP2C9/CYP3A4, primarily CYP2C9 in human interaction data. Do not imply equal clinical contributions of both enzymes.
Active metabolite About 14% of the oral dose converts; metabolite AUC about 4 times parent; potency 10–40 times by weight in pharmacology experiments. Exposure ratio is not a patient dose-conversion ratio.
Elimination Oral radiolabel: about 35% in urine and 60% in feces. About 4% of oral dose appears in urine unchanged and 6% as active metabolite. Radiolabel recovery includes metabolites; it is not all unchanged parent.
Clearance / accumulation Plasma clearance about 600 mL/min parent, 50 mL/min metabolite; renal clearance 75/25 mL/min. Neither accumulates with once-daily dosing in label studies. Context is studied regimens and populations, not every circumstance.
Renal impairment / dialysis Mild/moderate impairment raises parent/metabolite concentrations and AUC 50–90%; neither is removed by hemodialysis. Initial dose is not automatically adjusted for renal impairment alone; volume and safety assessment matter.
Hepatic impairment Mild/moderate alcoholic cirrhosis: parent exposure about 5 times and active metabolite about 1.7 times healthy controls; label start 25 mg. Severe impairment unstudied. This describes a labeled starting-dose context, not personal prescribing advice.
Interactions / clinical onset Fluconazole: metabolite AUC↓40%, parent↑70%; rifampin: parent↓30%, metabolite↓40%. Substantial BP response within 1 week, maximum 3–6 weeks. No universal compensatory dose change follows from these PK findings.

Landmark trials: keep the estimands distinct

Absolute differences below use crude observed participant counts: comparator proportion minus losartan proportion. Reciprocal values and upward-rounded integers are descriptive over each trial's observed follow-up, not adjusted fixed-time Kaplan–Meier NNTs. They do not transfer to another baseline risk or duration.

Trial and population Regimens / follow-up Results and interpretation
LIFE: 9,193 adults aged 55–80, hypertension plus ECG LVH Losartan-based n=4,605 versus atenolol-based n=4,588; common add-on HCTZ; mean 4.8 years Primary CV death/nonfatal MI/nonfatal stroke: 508/4,605 (11.03%) versus 588/4,588 (12.82%); crude ARR 1.785 percentage points, reciprocal 56.04, upward integer NNT 57. Adjusted relative risk 0.87 (95% CI 0.77–0.98).
LIFE secondary all-stroke endpoint Same population and follow-up Fatal/nonfatal stroke: 232/4,605 (5.04%) versus 309/4,588 (6.73%); crude ARR 1.697 points, reciprocal 58.93, upward integer NNT 59. Adjusted RR 0.75 (0.63–0.89). Not the first-event nonfatal-stroke component, whose counts are 209/286.
RENAAL: 1,513 participants with type 2 diabetes and nephropathy Losartan n=751 versus placebo n=762, added to other permitted BP therapy; mean 3.4 years Doubling of creatinine, ESRD or death: 327/751 (43.54%) versus 359/762 (47.11%); crude ARR 3.571 points, reciprocal 28.004, approximately 28 or strict upward integer NNT 29. Time-to-event risk reduction 16.1% (95% CI 2.3–27.9) in label analysis.
RENAAL components Same participants and follow-up ESRD 147/751 versus 194/762; crude ARR 5.885 points, upward integer NNT 17. Creatinine doubling 162/751 versus 198/762. Death 158/751 versus 155/762: no demonstrated reduction. Components overlap and must not be added.
ELITE: 722 adults ≥65 with NYHA II–IV HFrEF, EF≤40%, ACE-inhibitor naive Losartan n=352, target 50 mg once daily; captopril n=370, target 50 mg three times daily; 48 weeks Primary persistent creatinine rise ≥0.3 mg/dL: 10.5% in both groups, no observed absolute difference, no finite beneficial NNT. Exploratory mortality 4.8% versus 8.7% was unexpected and not a confirmatory survival result. Withdrawal for adverse effects 12.2% versus 20.8%.
ELITE II: 3,152 adults ≥60 with NYHA II–IV HFrEF, EF≤40% Losartan n=1,578 versus captopril n=1,574, same target doses as ELITE; median 555 days Average annual mortality 11.7% versus 10.4%; HR 1.13 (95.7% CI 0.95–1.35), p=0.16. No superiority demonstrated. These are annual rates, not cumulative risks, so no count-based ARR/NNT is calculated. Adverse-effect discontinuation excluding deaths 9.7% versus 14.7%; cough withdrawal 0.3% versus 2.7%.

LIFE; RENAAL; CANADA; ELITE; ELITE2; LABEL

LIFE's final mean BP was 144.1/81.3 versus 145.4/80.9 mmHg: similar, not equal. Cardiovascular mortality and MI separately were not significantly reduced. Label §1.2 says, “There is evidence that this benefit does not apply to Black patients.” In the subgroup, primary events were 46/270 (17%) versus 29/263 (11%); the cause and chance contribution remain uncertain. This limits the supported stroke-risk-reduction claim, not every use of losartan and not eligibility based on an inferred biological rule.

Patient natural frequencies — LIFE (two sentences): Over about five years, roughly 5 in 100 people on the losartan plan had a stroke, compared with about 7 in 100 on the atenolol plan. These people had high blood pressure and a thickened heart, and the label notes a limit to the stroke benefit in Black participants.

Patient natural frequencies — RENAAL (two sentences): Over about three and a half years, roughly 44 in 100 people on losartan had a major kidney problem or died, compared with 47 in 100 on placebo-based care. The study involved people with type 2 diabetes and kidney disease; it did not show fewer deaths on their own.

ELITE explanation (two sentences): In the first study, about 11 in 100 people in each group had the blood-test change that was the main outcome. The later, larger study did not show that losartan helped people live longer than captopril; its annual death rates cannot be read as counts out of 100 over the whole study.

Guideline placement

Guideline Specific recommendation and boundary
2025 AHA/ACC hypertension, §5.2.3 ARBs, ACE inhibitors, thiazide-type diuretics and long-acting dihydropyridine calcium-channel blockers are first-line classes. This is not proof that losartan is best for every patient. Exact class/level wording beyond this checked recommendation remains a release check.
2024 ESC elevated BP/hypertension The same four major classes form first-line options; this source concerns hypertension, not a newly inferred HFrEF hierarchy.
2022 AHA/ACC/HFSA HF ARNI for NYHA II–III HFrEF: Class 1A. ACE inhibitor when ARNI not feasible: Class 1A in chronic symptomatic HFrEF. ARB for previous/current symptoms of chronic HFrEF when ACE intolerance is due to cough or angioedema and ARNI is not feasible: Class 1A. Do not attribute this entire placement to ELITE II.
KDIGO 2024 §3.6 ACEi/ARB for G1–G4 CKD with A3 albuminuria without diabetes:1B; A2 without diabetes:2C; A2/A3 with diabetes:1B. Avoid ACEi/ARB/direct-renin-inhibitor combinations:1B.
KDIGO practice points3.6.1–3.6.5 Highest approved tolerated dose; check BP/creatinine/potassium within 2–4 weeks based on GFR and potassium. Manage hyperkalemia where possible rather than reflexively stopping. Continue unless creatinine rises >30% within 4 weeks of initiation/increase; symptomatic hypotension, uncontrolled hyperkalemia despite treatment, or selected end-stage circumstances require reassessment.

USHTN; ESC; HF; KDIGO

Creatinine thresholds are decision aids, not automatic reassurance. Evaluate illness, perfusion, NSAIDs, potassium and the broader clinical course. No patient should derive an individual dose or hold instruction from this table.

Urate: a second mechanism with bounded evidence

Established: URAT1, encoded by SLC22A12, handles renal urate reabsorption. Losartan inhibits this transport in experimental systems, and small clinical studies show increased urate clearance and lower serum urate. Würzner's 13-person crossover comparison found losartan reduced serum urate from 538±26 to 491±20µmol/L, while irbesartan did not; doubling the daily administration did not add a clear reduction in that study. Hamada compared losartan and candesartan in 32 hypertensive patients and included nine people with idiopathic hypouricemia; URAT1 function helped explain the response. These are not large gout-flare or renal-outcome trials.

Limits: Iwanaga's oocyte study found URAT1 inhibition with telmisartan as well, and effects varied with experimental conditions. Do not claim absolute biochemical exclusivity across every ARB, assume active metabolite and parent behave identically, or replace gout treatment with losartan on this basis.

URAT; URATECLIN

Mechanism-based harms and checks

Mechanism Risk / practical question
Less aldosterone Potassium can rise; reconcile supplements, potassium salt substitutes and potassium-sparing drugs.
Less angiotensin-dependent filtration pressure Creatinine can rise or AKI occur, especially with volume loss, renal artery disease, CKD or severe HF; evaluate cause and trend.
Reduced vasoconstriction Symptomatic hypotension, especially in salt/volume depletion.
Fetal RAS blockade Boxed fetal injury/death warning; the label calls for discontinuation as soon as pregnancy is detected and prompt clinical action.
Hypersensitivity Contraindicated with hypersensitivity to any component; swelling affecting breathing requires urgent care.
Dual RAS blockade Aliskiren with losartan is contraindicated in diabetes; combinations generally increase renal, potassium and hypotension risks.
No direct ACE inhibition Less characteristic ACE-related cough, not zero cough or zero angioedema.

The Cozaar label describes VA NEPHRON-D: 1,448 participants with diabetic albuminuric CKD received background losartan with lisinopril or placebo; no additional primary benefit and more AKI/hyperkalemia occurred. Its historical dual-blockade regimen is not a recommendation. Tablet potassium content is 2.12/4.24/8.48 mg for 25/50/100 mg strengths; hormonal renal effects explain the main hyperkalemia risk.

Class comparison: chemistry and evidence are different axes

Agent Active form / half-life What can be compared responsibly
Losartan Active parent plus EXP3174 (E-3174); about 2 h/6–9 h LIFE/RENAAL are population-specific; small human urate comparisons support an additional pharmacodynamic effect.
Valsartan Active parent; about 6 h Label includes hypertension, HF and post-MI evidence contexts. Its trial evidence is not a head-to-head efficacy ranking against every ARB.
Olmesartan medoxomil Ester prodrug→olmesartan; about 13 h Hypertension label; ROADMAP's microalbuminuria endpoint must not be recast as proven kidney-failure or survival benefit.
Telmisartan Active parent; about 24 h ONTARGET provides a different high-risk population/comparator context. Experimental URAT1 inhibition does not establish the same clinical urate effect as losartan.
Candesartan cilexetil Ester prodrug→candesartan; about 9 h CHARM HF programs support distinct outcome claims; prodrug status and half-life do not establish superiority.
Lisinopril Active ACE inhibitor; effective accumulation half-life about 12 h Acts upstream and affects bradykinin degradation; review its own HF/post-MI/HTN evidence and cough/angioedema risks.
Sacubitril/valsartan Neprilysin inhibitor plus ARB Separate combination and HFrEF guideline role; not an interchangeable single-agent ARB comparison.

How it was discovered

Losartan grew from medicinal-chemistry efforts to block angiotensin II with an orally useful, nonpeptide molecule. Duncia and colleagues reported in 1990 that adding an aromatic ring and placing acidic and lipophilic groups appropriately improved antagonist activity. Carini’s 1991 work developed the biphenyl imidazole series and described the series containing tetrazole-bearing DuP 753, later called losartan. The parent was active, and Wong’s 1990 experiments characterized its more potent carboxylic acid metabolite, EXP3174 (E-3174). The US label records initial approval in 1995. RENAAL and LIFE subsequently tested kidney and cardiovascular outcomes in defined populations, moving beyond blood-pressure measurements alone. Teva announced generic approval in 2010. Structural research later supplied molecular views of receptor antagonism, including a losartan-bound complex reported before the 2026 engineered 9ZXC structure. This history separates chemical discovery, regulatory approval, clinical outcomes and structural explanation; none is a substitute for the others.

Patient

Before (actual dossier): Losartan blocks a receptor that receives a blood-vessel-tightening signal called angiotensin II. Blood vessels relax and salt-and-water hormone signals change, helping lower blood pressure.

After (saved perspective draft): Losartan blocks AT1, a switch that a hormone uses to squeeze blood vessels.

FK before 11.71; after 5.07.

Caregiver

Before (actual dossier): Losartan stops angiotensin II from delivering much of its vessel-tightening message. The effect is assessed with blood-pressure readings and follow-up, not whether the person immediately feels different.

After (saved perspective draft): Losartan and its active form, EXP3174 (E-3174), block AT1 switches in blood vessels, kidneys and the adrenal glands.

FK before 13.28; after 5.95.

Pharmacist

Before (actual dossier): Losartan and its active metabolite block AT1 receptors, reducing angiotensin-II-driven vasoconstriction and aldosterone release. Unlike ACE inhibitors, they do not directly inhibit ACE or bradykinin degradation.

After (saved perspective draft): Losartan and EXP3174 (E-3174) block vascular, adrenal and renal AT1 signaling to lower pressure.

FK before 15.80; after 14.65.

Clinician

Before (actual dossier): AT1 antagonism by losartan and its active metabolite reduces vasoconstrictor and aldosterone responses to angiotensin II. The pathway remains blocked despite feedback increases in renin and circulating angiotensin II.

After (saved perspective draft): Losartan and EXP3174 (E-3174) antagonize vascular, adrenal and renal AT1 signaling.

FK before 16.51; after 12.50.

Nurse

Before (actual dossier): Losartan blocks angiotensin-II signals that tighten vessels and promote aldosterone release. Blood pressure, kidney function and potassium help assess consequences of this pathway, not just symptoms.

After (saved perspective draft): Losartan and EXP3174 (E-3174) block AT1 responses in vessels, adrenal glands and kidneys.

FK before 10.81; after 11.91.

Student

Before (actual dossier): Angiotensin II activates AT1 receptors to promote vasoconstriction and aldosterone release. Losartan blocks the receptor, rather than the ACE enzyme that produces angiotensin II.

After (saved perspective draft): Losartan and EXP3174 (E-3174) antagonize AT1 receptors in vascular smooth muscle, adrenal tissue and the kidney.

FK before 13.67; after 15.43.

Scientist

Before (actual dossier): Losartan and its active metabolite antagonize AT1 signaling without inhibiting ACE. Separate receptor antagonism from feedback increases in renin; rising upstream hormone concentrations do not alone establish loss of blockade.

After (saved perspective draft): Losartan and EXP3174 (E-3174) antagonize AT1 signaling in vascular, adrenal and renal tissues, lowering blood pressure.

FK before 16.22; after 15.30.

Chemist

Before (actual dossier): Losartan is an AT1 antagonist with a more potent active metabolite. Binding studies distinguish competitive parent antagonism from the metabolite's different functional behavior; neither acts by ACE inhibition.

After (saved perspective draft): Losartan potassium combines an acidic biphenyl tetrazole with a 2-butyl-4-chloro-imidazole bearing a 5-hydroxymethyl group. Oxidation yields the carboxylic acid EXP3174 (E-3174), alongside an already active parent. Both antagonize AT1 in vessels, adrenal glands and kidneys, lowering pressure. Duncia/Carini SAR supports the ortho acidic group and lipophilic substituents; current human interaction data primarily implicate CYP2C9 in metabolite formation. Skiba 2026 and PDB 9ZXC place ligand LSN in an engineered receptor complex at 3.4Å. The potassium counterion describes the salt; elevated serum potassium follows chiefly from altered renal hormonal physiology.

FK before 15.16; after 14.67.

Investor / Diligence Analyst

Before (actual dossier): Losartan interrupts angiotensin-II signaling at AT1 receptors, lowering vascular and aldosterone responses. Its known target does not establish interchangeability of formulations or clinical outcomes across populations.

After (saved perspective draft): Losartan and EXP3174 (E-3174) block vascular, adrenal and renal AT1 signaling, supporting a mature blood-pressure franchise.

FK before 16.71; after 10.92.

General

Before (actual dossier): Losartan blocks the receptor for a signal that narrows blood vessels. Blocking that message helps vessels relax; it does not stop the body from making the signal.

After (saved perspective draft): Losartan blocks AT1, the receptor that receives a vessel-tightening message from angiotensin II.

Public FAQ reading-level checks

FAQ Before FK After FK Check
Patient 05 — How will I know whether it is helping? 8.67 5.76 Revised answer is below 8; older answer exceeded 8.
Patient 18 — Will I need tests or monitoring? 14.27 6.72 Revised answer is below 8; older answer exceeded 8.
Caregiver 11 — device help Not applicable 6.78 Dossier marks this item not applicable; draft correctly explains tablets and the labeled liquid-measuring context.
Nurse 07 — dilution, infusion or compatibility Not applicable 15.20 Dossier marks this item not applicable; draft explicitly limits non-applicability to IV checks and retains oral-liquid safeguards.
General 10 — How was losartan discovered? 14.74 8.54 New answer is in the 8–10 target; it adds a sourced discovery chronology beyond the old unresolved placeholder.

Patient 05 — contextual check

Before (actual dossier): For losartan, look for the treatment goal agreed with the clinician: blood-pressure control and, in diabetic nephropathy, longer-term kidney and urinary-protein trends. You may feel no different despite benefit. Keep a record rather than judging one dose.

After (saved FAQ repair): A series of blood-pressure readings shows more than how you feel after one pill.

FK before 8.67; after 5.76. The revision uses concrete repeated readings, a realistic 1–6 week response window, an agreed treatment goal, kidney follow-up only for some people with diabetes and kidney disease, and a reminder not to self-adjust after one high reading.

Patient 18 — contextual check

Before (actual dossier default answer): Follow blood pressure, creatinine or estimated filtration, and potassium after initiation and dose changes, then according to clinical risk. In diabetic nephropathy, urinary protein and renal trends help assess the condition over time. Report vomiting, diarrhea, poor intake, NSAID use, or symptomatic low pressure because these can change renal risk.

After (saved FAQ repair): Blood tests check potassium, a salt in the blood, and creatinine, a sign of kidney function.

FK before 14.27; after 6.72. The revision explains potassium and creatinine in everyday terms, gives the risk-adjusted 2–4 week check timing for chronic kidney disease, and names fluid loss and NSAIDs as reasons to contact the care team.

Non-applicable role questions

The dossier records caregiver-11 and nurse-07 as not-applicable, not as missing answer text. Their final repairs explain why device/infusion assumptions do not directly apply to losartan while retaining formulation-specific liquid, route, measuring and storage safeguards. These checks assess whether the prompt was handled contextually; they do not add a clinical review.

General FAQ other-10

Before (actual dossier): A detailed, verified account of the original losartan discovery program is not established in this review. The label records initial US approval in 1995, which is a regulatory milestone rather than a discovery date. A discovery narrative should be added only after checking original medicinal-chemistry or development records.

After (saved FAQ repair): Losartan came from a search for small molecules that could block the angiotensin II receptor.

FK before 14.74; after 8.54. The revised answer is within the General target range.

Readability decisions

Patient introduction grades: 5.07; Caregiver: 5.95; General: 8.87. Patient/Caregiver drafts are below the 8 ceiling. General is within the required 8–10 range. Pharmacist, Clinician, Nurse, Student, Scientist, Chemist and Investor/Diligence Analyst are technical audiences and are not held to the patient ceiling. No comprehension test or clinical review is claimed.

Fact-preservation check: the patient and caregiver drafts explain AT1/receptor action in familiar terms while retaining the active metabolite, expected onset/full-effect range, blood-test context and pregnancy/contraindication warnings. The specified FAQ repairs distinguish tablet/device and IV prompts from losartan’s oral route while preserving labeled liquid precautions.

Decisions made for the founder

  • The audit's “no cough,” “equal blood pressure,” “first picture ... only in2026,” universal class-unique URAT1 and “current supply is tested” wording exceeds the supporting evidence. Replace with the narrower documented claims; this is correction of the audit, not a substantiated missed live safety error.
  • Use current Cozaar “about1000-fold” AT1/AT2 affinity wording. In-vitro CYP2C9/3A4 participation is distinct from human interaction evidence primarily implicating CYP2C9.
  • RENAAL's rounded percentages imply approximately28 treated per observed composite event avoided; exact counts give reciprocal28.004 and strict upward integer29. Show the arithmetic and distinguish crude risk differences from adjusted time-to-event effects. ELITE II annual rates cannot yield a cumulative NNT.
  • Do not turn a nonsignificant superiority result into equivalence or inferiority. Do not attribute present HF placement solely to ELITE II. State guideline eligibility conditions.
  • The LIFE label limitation concerns the supported stroke-risk-reduction indication in hypertension plus LVH. Preserve that limitation without treating it as a contraindication or extending it to every use or outcome.
  • KDIGO's creatinine threshold does not authorize ignoring symptomatic hypotension, hyperkalemia, volume depletion or acute kidney injury.
  • Keep the selector, useful existing Gen1 answers and exact-answer precedence. The two status objects receive readable, route-specific answers. Existing strong safety/caregiver answers remain available.
  • Numeric prices that expired October16 are withdrawn from the proposed implementation. No current price, IP clearance or supplier quality assurance is invented.
  • Apply lower-model subagents to structure/index QA, visual/translation drafting and language measurements; primary-agent review retains difficult evidence interpretation. Drafts remain local; no publication, clinical sign-off or outreach occurred.

Translation draft — native clinical review pending

Scope: a Spanish patient-facing draft aligned to the exact English Patient introduction in work/losartan/03-perspectives.md and the revised patient FAQ answers patient-05 and patient-18 in work/losartan/04-faq-by-perspective.md. The additional safety box carries the Cozaar label's boxed fetal-toxicity warning and both contraindications. This is translation/editorial work, not clinical validation, a prescribing instruction, or an approved localized site page. Use the same source and review badges as the English draft.

Locked English source: Patient introduction from 03

Back-translation of the Spanish introduction for meaning check: “Losartan blocks the AT₁ receptor, like a switch a hormone uses to narrow blood vessels. The body also produces an active form called EXP3174 (E-3174), which helps block the receptor. Vessels can relax, and signals regulating salt in the kidneys change. Blood pressure begins to fall in the first week; full effect may take 3–6 weeks. Blood tests check kidney function and potassium. The highlighted warning says the drug can harm or kill a developing baby. The label prohibits use with allergy to any ingredient, or with aliskiren if the person has diabetes. Discuss these risks with the care team.” No English clinical claim is intentionally added in this paragraph. The safety box separately states the label-directed urgent pregnancy action.
