Apixaban (Eliquis)

Adult US Eliquis oral film-coated tablets. Pediatric products and individualized regimens are outside this page’s scope.

Educational draft · source check October 10, 2026 · independent clinical review pending.

Uses / Symptoms

  • Reduces stroke risk in nonvalvular atrial fibrillation.
  • Treats DVT or pulmonary embolism; reduces recurrence after initial treatment. Also prevents DVT after hip or knee replacement.

Effects

  • Reduces clot formation by inhibiting factor Xa.

Side effects

  • Easier bruising or bleeding; serious bleeding can be fatal.

Selected uses and effects; not a complete safety list. Eliquis prescribing information and Medication Guide.

Apixaban blocks factor Xa, a protein that helps your blood make clots. Less clotting activity helps prevent harmful clots, but also makes bleeding easier; it does not directly dissolve a clot.

How it works

After an oral tablet is taken, apixaban is absorbed into the body. Its molecular target is factor Xa, an enzyme in the blood-clotting system. It inhibits both free factor Xa and factor Xa associated with a clot.

Blocking factor Xa reduces the generation of thrombin and the development of blood clots. This helps explain its use to prevent certain clots and to treat deep-vein thrombosis or pulmonary embolism. Its direct effect is on the clotting enzyme rather than platelet aggregation.

The same reduction in clotting creates a bleeding risk, sometimes serious or fatal. Stopping treatment prematurely can increase clot risk. Treatment interruptions therefore require a clinician-directed plan, rather than being an automatic response to feeling better.

Questions about apixaban

Patient-focused answers. These are shared factual explanations, not personalized treatment advice.

Why is it prescribed?

Adult uses include reducing stroke risk in nonvalvular atrial fibrillation, treating DVT or pulmonary embolism, reducing recurrence after initial treatment, and preventing DVT after hip or knee replacement.

What does it block?

Factor Xa, reducing thrombin generation and clot development.

Is it a platelet blocker?

It has no direct effect on platelet aggregation; its primary target is factor Xa.

What is the main side effect?

Bleeding, which can be serious or fatal.

Can I stop when I feel better?

Do not stop without speaking to your prescriber; premature discontinuation raises clot risk.

What if unusual bleeding occurs?

Seek urgent medical help for severe or uncontrolled bleeding, blood in vomit or stool, or red/brown urine. Get prompt medical advice after a fall or head injury. These are examples, not a complete list of warning signs.

MedlinePlus · precautions and serious symptoms

Can other medicines increase bleeding?

Yes, including aspirin, anti-inflammatory pain medicines such as ibuprofen, and other anticoagulants. Review new medicines with your clinician or pharmacist.

What about surgery or dental work?

Tell the treating team you take apixaban. They must plan any interruption.

Why is there a spinal-procedure warning?

Spinal or epidural bleeding can cause lasting paralysis; the procedure team needs to know about treatment.

Does this page cover children?

No. Pediatric treatment uses separate weight-based instructions and presentations.

How is it taken?

Adult tablets are taken by mouth, with or without food. The dose and treatment duration depend on the indication and individual factors. Follow the prescription; the animation is not a dosing guide.

Eliquis label · 2 DOSAGE AND ADMINISTRATION

What if I miss a dose?

The Eliquis label instructs taking a missed dose as soon as possible on the same day, then resuming the twice-daily schedule. Do not double a dose to make up for a missed one. Ask your pharmacist if the timing is unclear.

Eliquis label · 2.3 Missed Dose

Does it require the same blood test as warfarin?

Apixaban affects clotting tests, but changes in PT, INR and aPTT are small and variable and are not useful for monitoring its anticoagulant effect. This does not mean follow-up is unnecessary; keep the appointments and tests your clinician recommends.

Eliquis label · 12.2 Pharmacodynamics

Can it interact with more than blood thinners?

Yes. Strong combined P-gp and CYP3A4 inhibitors can raise apixaban exposure; strong inducers can lower it. The label names examples including ketoconazole, ritonavir, rifampin, carbamazepine and St. John’s wort. Let your pharmacist check the full list rather than changing doses yourself.

Eliquis label · 7 DRUG INTERACTIONS

What about pregnancy or breastfeeding?

Discuss pregnancy plans with your prescriber. Pregnancy can increase bleeding-related concerns, and the label does not recommend use during pregnancy. Breastfeeding is not recommended during treatment. A clinician should discuss suitable options.

Eliquis label · 8 USE IN SPECIFIC POPULATIONS

What if too much is taken?

An overdose increases bleeding risk. Obtain urgent medical or poison-control advice. Do not attempt reversal or adjust other medicines yourself; emergency treatment depends on the situation.

Eliquis label · 10 OVERDOSAGE

What trials support its uses?

The label describes ARISTOTLE for atrial fibrillation, ADVANCE studies after hip or knee replacement, AMPLIFY for DVT/PE treatment, and AMPLIFY-EXT for recurrence prevention. Different indications have different study populations and comparators; the GIF does not estimate your personal benefit.

Eliquis label · 14 CLINICAL STUDIES

Why can the benefit be hard to feel?

Preventing a stroke or another clot is a risk-reduction goal, not necessarily a symptom you can feel each day. Feeling well does not establish that treatment is no longer needed.

Eliquis label · 1 INDICATIONS AND USAGE

How should tablets be stored?

The US label specifies controlled room-temperature storage, 20–25°C (68–77°F), with permitted excursions to 15–30°C (59–86°F). Keep medicines out of children’s reach and follow the dispensing label.

Eliquis label · 16 HOW SUPPLIED/STORAGE AND HANDLING

Who needs additional precautions?

The label contraindicates use in active pathological bleeding or severe hypersensitivity to apixaban. It does not recommend apixaban for prosthetic heart valves or triple-positive antiphospholipid syndrome. Discuss kidney or liver disease, pregnancy, breastfeeding and all medicines with your treating team.
